Logo.jpgTo register with the surgery in person.  Please bring your medical card, if you have one. You will be given a registration form to complete. We also require two forms of identification, one photo identification (for example passport or photo driving licence) and proof of address such as a utility bill or bank statement dated within the last three months.

Please note that Primary Care Support Service no longer routinely issue medical cards on registering with a practice.  Reception staff will be able to confirm your NHS number if required once we have received proof of identity.

Our reception staff will be pleased to book you a New Patient Check and answer any queries you may have.
